a) Passport Size (35x35mm) Photograph with light background and frontal view. (09 - Photos)

b)  Original Address Proof( Telephone Bill/Electricity bill/Ration Card/Passbook/ Election card with 02 Self attested photocopies.

c)  Original Proof of Age

(Birth Certificate/10th Marksheet) with 02 Self attested photocopies.


d)  Original Graduation or Professional degree

/ Last Three Years IT return for ECNR with 02 Self attested photocopies.


e)  NOC from Deptt ( Incase of Govt. employee)


f)  Affidavit with joint photograph and two Paper cutting (In case of Name Change).
